A CEO logs into their corporate Office 365 portal. A session cookie is saved. Malware steals it. The hacker loads that cookie into their Firefox browser, opens Outlook, and starts sending wire transfer requests to finance. The CEO never receives a login alert because the hacker didn't "log in"—they resumed an existing session.
